What the buyer wants

  1. The object of the order is to carry out the investment task of "Modernization of premises in the building of the locker room of the Municipal Stadium at 28" Dworcowa Street. The task includes a comprehensive modernization of the rooms of the building in the locker room of the City Stadium at 28 Dworcowa Street in Knurów (d. 2141/59). As part of the task, a new technical infrastructure will be performed, i.e.: electrical installation, water-channel installation, mechanical ventilation and air conditioning (in selected rooms).
  2. The scope of the order includes, among others:
  1. execution of preparatory, dismantling and demolition works,
  2. completion of a comprehensive modernization of the locker room, sanitary rooms and halls in the building,
  3. execution of a new electrical and low current installation (teletechnical network),
  4. execution of a new water and sewage and sanitary installation,
  5. installation of mechanical ventilation installations,
  6. replacement of stair rails in corridors,
  7. execution of a new door joinery and entrance doors to the building,
  8. purchase of necessary equipment of the changing rooms.
  9. The detailed scope of the subject-matter of the contract shall be determined by the design documentation and technical specifications for the execution and reception of works, pre-measurement of works and arrangements resulting from the model contract, which shall be annexed to the SWI. The contractor is obliged to consult the SWZ and its annexes and to complete the order under the terms and conditions described therein.
  10. The Contracting Authority shall allow the use of devices and equivalent materials wherever it describes in the design documentation the subject matter of the contract by means of trademarks, patents or origin, source or specific process which characterises the products or services supplied by a particular Contractor. The type of equipment, catalogue cards, descriptions, drawings and producer names should be considered as examples.
  11. All parameters, including technical – operational and usable parameters of the equipment and materials used by the Contractor in the execution of the order must comply with the requirements set out in the SWZ and its annexes. The change of any of the devices, components, materials, etc. shall take into account all technical parameters that are relevant to the operation of the object as a whole, as well as specific architectural and structural limitations.
  12. The Contractor will be obliged to grant a quality guarantee on the subject of the contract (work and materials) for a period declared in the offer, not less than 36 months. The term of the guarantee shall begin on the date of signature of the final receipt protocol for the subject matter of the contract.

  1. O udzielenie zamówienia mogą się ubiegać Wykonawcy, którzy nie podlegają wykluczeniu z udziału w postępowaniu w okolicznościach wskazanych:
  1. w art. 108 ust. 1 PZP oraz w art. 109 ust. 1 pkt 1) i pkt 4) PZP, z zastrzeżeniem art. 110 ust. 2 PZP
  2. w art. 7 ust. 1 ustawy z dnia 13 kwietnia 2022 r. o szczególnych rozwiązaniach w zakresie przeciwdziałania wspieraniu agresji na Ukrainę oraz służących ochronie bezpieczeństwa narodowego.
  3. Wykluczenie Wykonawcy może nastąpić na każdym etapie postępowania o udzielenie zamówienia.
  4. O udzielenie zamówienia mogą się ubiegać Wykonawcy, którzy spełniają warunki udziału w postępowaniu dotyczące: 2) zdolności do występowania w obrocie gospodarczym: Zamawiający nie określa warunku udziału w postępowaniu w tym zakresie.
  5. uprawnień do prowadzenia określonej działalności gospodarczej lub zawodowej, o ile wynika to z odrębnych przepisów: Zamawiający nie określa warunku udziału w postępowaniu w tym zakresie.
  6. sytuacji ekonomicznej lub finansowej: Wykonawca spełni warunek udziału w postępowaniu, jeżeli wykaże, że jest ubezpieczony od odpowiedzialności cywilnej w zakresie prowadzonej działalności związanej z przedmiotem zamówienia, na sumę gwarancyjną co najmniej 2.000.000,00 zł.
  7. zdolności technicznej lub zawodowej*: Wykonawca spełni warunek udziału w postępowaniu, jeżeli wykaże się:
    1. doświadczeniem w okresie ostatnich 5 lat przed upływem terminu składania ofert, a jeżeli okres prowadzenia działalności jest krótszy - w tym okresie, w wykonaniu co najmniej: a.1) co najmniej dwóch robót budowlanych o wartości brutto min. 1.000.000,00 zł każda, polegających na budowie, przebudowie lub remoncie budynku wraz z instalacjami wod.-kan. i/lub centralnego ogrzewania i/lub elektryczną i/lub wentylacyjną i/lub klimatyzacyjną. Warunek, o którym mowa wyżej zostanie spełniony jeżeli jeden podmiot wykaże, że posiada doświadczenie w wykonaniu co najmniej dwóch robót budowlanych opisanych powyżej.
    2. dysponowaniem osobami, zdolnymi do wykonywania zamówienia, na stanowiska: b.1) Kierownika budowy - osobą posiadającą uprawnienia budowlane do kierowania robotami budowlanymi w specjalności konstrukcyjno – budowlanej oraz minimum 3-letnie doświadczenie zawodowe w kierowaniu robotami, po uzyskaniu uprawnień budowlanych, b.2) kierownika robót elektrycznych - osobą posiadającą uprawnienia budowlane do kierowania robotami budowlanymi w specjalności instalacyjnej w zakresie instalacji i urządzeń elektrycznych i elektroenergetycznych oraz minimum 3-letnie doświadczenie zawodowe w kierowaniu robotami, po uzyskaniu uprawnień budowlanych, b.3) kierownika robót sanitarnych - osobą posiadającą uprawnienia budowlane do kierowania robotami budowlanymi w specjalności instalacyjnej w zakresie sieci, instalacji i urządzeń cieplnych, wentylacyjnych, wodociągowych i kanalizacyjnych oraz minimum 3-letnie doświadczenie zawodowe w kierowaniu robotami, po uzyskaniu uprawnień budowlanych.W przypadku zaistnienia konieczności prowadzenia prac budowlanych w innych branżach Wykonawca zapewni dodatkowe osoby dla każdej z branż, posiadające odpowiednie uprawnienia.Osoby przewidziane do pełnienia samodzielnych funkcji technicznych w budownictwie muszą posiadać wymagane uprawnienia, które zostały wydane zgodnie z ustawą Prawo budowlane z dn. 7 lipca 1994 r. art. 12 ust. 1 ppkt 2), ust. 7, art. 12a lub uprawnienia uzyskane na podstawie przepisów obowiązujących przed wejściem w życie ustawy Prawo budowlane z 1994 r., natomiast w przypadku osób, które uzyskały uprawnienia w innych krajach Unii Europejskiej, a także osób z Europejskiego Obszaru Gospodarczego (tj. Królestwo Norwegii, Księstwo Liechtensteinu, Republika Islandii) i obywateli Konfederacji Szwajcarskiej, posiadane uprawnienia muszą spełniać warunki określone w Ustawie z dn. 22 grudnia 2015 r. o zasadach uznawania kwalifikacji zawodowych nabytych w państwach członkowskich Unii Europejskiej (tekst jedn. Pełnomocnik może być ustanowiony do reprezentowania Wykonawców w postępowaniu albo do reprezentowania w postępowaniu i zawarcia umowy.
  6. oświadczenia, o których mowa w art. 125 ust. 1 PZP Wykonawcy / Wykonawców wspólnie ubiegających się o udzielenie zamówienia / Podmiotu udostępniającego zasoby / Podwykonawcy, sporządzone na Załączniku nr 1) do „Formularza ofertowego” oraz podpisane przez osoby upoważnione do reprezentacji odpowiednio Wykonawcy / Wykonawców wspólnie ubiegających się o udzielenie zamówienia / Podmiotu udostępniającego zasoby / Podwykonawcy,
  7. zobowiązanie podmiotów udostępniających zasoby lub inny podmiotowy środek dowodowy potwierdzający, że Wykonawca realizując zamówienie, będzie dysponował niezbędnymi zasobami tych podmiotów, wymagane postanowieniami Rozdziału 4 ust. 9 SWZ, w przypadku, gdy Wykonawca polega na zdolnościach innych podmiotów w celu potwierdzenia spełniania warunków udziału w postępowaniu.   8. oświadczenie, o którym mowa w art. 117 ust. 4 PZP, Wykonawców wspólnie ubiegających się o udzielenie zamówienia, w zakresie określonym w Rozdz. 4 ust. 6 SWZ, z którego wynika, które roboty budowlane lub usługi wykonają poszczególni Wykonawcy. Wzór oświadczenia stanowi Załącznik nr 3) do „Formularza ofertowego”,
  9. wadium – dokument (jeżeli jest wymagane),
  10. ewentualne zastrzeżone informacje stanowiące tajemnicę przedsiębiorstwa w rozumieniu przepisów o zwalczaniu nieuczciwej konkurencji wraz z uzasadnieniem – zgodnie z zasadami wskazanymi w Rozdziale 8 ust. 4 pkt 7) SWZ. Wykonawca nie może zastrzec informacji, o których mowa w art. 222 ust. 5 PZP.
  11. Składanie dokumentów na wezwanie:
  1. Wykonawca, który polega na zdolnościach podmiotów udostępniających zasoby na zasadach określonych w art. 118 PZP, zobowiązany jest przedstawić w odniesieniu do tych podmiotów dokumenty wymienione w Rozdz. 5 ust. 2 pkt 1) lit. b) do d) SWZ.
  2. Wykonawca, który zamierza powierzyć wykonanie części zamówienia wskazanych w ofercie Podwykonawcom, nie będących podmiotami udostępniającymi zasoby na zasadach określonych w art 118 PZP, zobowiązany jest przedstawić w odniesieniu do tych podmiotów dokumenty wymienione w Rozdz. 5 ust. 2 pkt 1) lit. b) do d) SWZ.
  3. W przypadku Wykonawców wspólnie ubiegających się o zamówienie dokumenty wymienione w Rozdz. 5 ust. 2 pkt 1) lit. a) do d) SWZ składa każdy z tych Wykonawców.
