Subsequent evidence of suitability shall be submitted. (With the offer; by means of self-declaration): suitability for professional practice: - Business registration - Proof of registration in a professional or commercial register or proof in another way of the permitted professional practice Economic and financial capacity: - Turnover of the company in the last three completed financial years - Proof of professional or company liability insurance - Health insurance certificates - Health insurance certificate of the professional association - Health insurance certificate of the tax office or Certificate in tax matters, if the tax office issues such a certificate Technical and professional capacity: - List of main services provided in the last three years with indication of the value, the period of service provision and the client - Declaration on the number of employees employed in the last 3 years on average per year, broken down by wage group, with specially designated management staff other conditions of suitability: indication of the PQ number, if a prequalification is available. Prequalified companies provide proof of eligibility by entering in the Official List of Prequalified Companies for the Supply and Service Sector (Prequalification List). Non-prequalified companies must provide the completed form "Self-declaration of eligibility" together with the above-mentioned proofs of eligibility with the offer.
