What the buyer wants
1. The subject of the contract entitled: "Purchase of computer hardware" is the delivery of computer hardware meeting the quality parameters specified in the description of the subject of the contract - Annex No. 3 to the SWZ, under the conditions specified in the draft contractual provisions constituting Annex No. 10 to the SWZ, covering within its scope: 1) Computer workstation type 1 - 24 pcs. 2) Computer workstation type 2 - 1 pc. 3) Laptop computer - 1 pc. 4) Monitor - 5 pcs. CONTRACT EXECUTION DEADLINE Start date: from the date of contract conclusion Completion date: up to 21 days from the date of contract conclusion List of subjective evidence 1. The Contractor whose tender is rated the highest, in order to demonstrate compliance with the conditions for participation in the procedure (specified by the Contracting Authority in Section 3 of this chapter of the SWZ), will be requested pursuant to Art. 126(1) of the Act to submit the following subjective evidence (current as of the date of submission): 1.1. a list of deliveries (in accordance with Annex No. 5 to the SWZ) performed, and in the case of recurring or continuous services also being performed, respectively during the last 3 years, and if the period of business activity is shorter – within this period, together with their value, subject matter, dates of performance, and entities for which these deliveries were performed or are being performed, along with attached evidence specifying whether these deliveries were performed or are being performed properly, 1.2. information from the National Criminal Register within the scope of Art. 108(1), (2), and (4) of the Public Procurement Act, drawn up no earlier than 6 months prior to its submission, 1.3. a statement of the Contractor within the scope of Art. 108(1)(5) of the Public Procurement Act on the absence of membership in a capital group within the meaning of the Act of 16 February 2007 on Competition and Consumer Protection (consolidated text: Journal of Laws of 2024, item 1616) with another Contractor who submitted a separate tender, partial tender, or application to participate in the procedure independently of another Contractor belonging to the same capital group - a template of the statement constitutes Annex No. 7 to the SWZ; 1.4. a statement of the Contractor / Third Party providing resources on the current validity of the information contained in the statement referred to in Art. 125(1) of the Public Procurement Act, regarding the grounds for exclusion from the procedure indicated by the Contracting Authority, as referred to in Art. 108(1) of the Public Procurement Act - a template of the statement constitutes Annex No. 6 to the SWZ. In the case of Contractors jointly applying for the award of the contract, the subjective evidence referred to in points 1.2, 1.3, and 1.4 shall be submitted by each of the Contractors acting jointly. Contractors may jointly apply for the contract award. 1. Contractors jointly applying for the award of the contract shall appoint an attorney (proxy) to represent them in the procurement procedure or to represent them in the procedure and conclude the public procurement contract – this does not apply to a civil law partnership (spółka cywilna), provided that the authorization/power of attorney to act on behalf of this partnership arises from the partnership agreement attached to the tender or all partners sign the tender. 2. Contractors jointly applying for the contract award are required to submit an appropriate power of attorney along with the tender – (not applicable to a civil law partnership, provided that the authorization/power of attorney to act on behalf of this partnership arises from the partnership agreement attached to the tender or all partners sign the tender). The power of attorney may arise either from a document with such a title or from the agreement of the Contractors jointly applying for the award of the contract. 3. Przedmiotem zamówienia pn.: „Zakup sprzętu komputerowego” jest dostawa sprzętu komputerowego spełniającego parametry jakościowe określone w opisie przedmiotu zamówienia - Załączniku nr 3 do SWZ, na warunkach określonych w projektowanych postanowieniach umowy stanowiących Załącznik nr 10 do SWZ, obejmujący swoim zakresem: 1) Komputerowa stacja robocza typ 1 - 24 szt. 2) Komputerowa stacja robocza typ 2 - 1 szt. 3) Komputer typu laptop - 1 szt. 4) Monitor - 5 szt. 2. Zamawiający zastrzega sobie prawo do jednostronnego w ramach prawa opcji rozszerzenia zamówienia o dodatkowe urządzenia: 1) Komputerowa stacja robocza typ 1 - maksymalny zakres dodatkowej dostawy 5 szt. 2) Komputerowa stacja robocza typ 2 - maksymalny zakres dodatkowej dostawy 1 szt. 3) Komputer typu laptop - maksymalny zakres dodatkowej dostawy 1 szt. 4) Monitor - maksymalny zakres dodatkowej dostawy 7 szt. 3. Dostawa przedmiotu zamówienia wraz z transportem, rozładunkiem i wniesieniem do siedziby Zamawiającego lub do innego miejsca wskazanego przez Zamawiającego na terenie Gminy Czechowice-Dziedzice będzie się odbywać na koszt i ryzyko wykonawcy. 4. Dostarczone urządzenia muszą pochodzić z oficjalnego kanału dystrybucyjnego na terenie Polski. 5. Wszystkie rozwiązania dostarczone w ramach zamówienia muszą być fabrycznie nowe i nieużywane przed dniem dostarczenia do siedziby Zamawiającego, z wyłączeniem użycia niezbędnego dla przeprowadzenia testu ich poprawnej pracy. Nie dopuszcza się urządzeń i licencji typu refurbished. 6. Dostarczone urządzenia muszą być wolne od wad fizycznych i prawnych. 7. Zamawiający zastrzega sobie prawo do jednostronnego w ramach prawa opcji rozszerzenia zamówienia. 8. W przypadku skorzystania przez Zamawiającego z prawa opcji wykonawca jest zobowiązany do jego realizacji, na warunkach określonych w umowie. 9. Zamawiający może skorzystać z prawa opcji w terminie 7 dni od zawarcia umowy, poprzez jednostronne oświadczenie woli przekazane wykonawcy w formie elektronicznej. 10. Na cały przedmiot zamówienia zostanie udzielona gwarancja producenta na okres minimum 60 miesięcy. 11. Zamówienie finansowane ze środków własnych Zamawiającego. 12. Przedmiotowe środki dowodowe: Zamawiający wymaga złożenia przedmiotowych środków dowodowych, które będą podlegać uzupełnieniu zgodnie z zapisami SWZ: 1) dokumenty (katalog, folder lub dokumentacja techniczna) pochodzące od producenta oferowanych urządzeń, zawierających parametry jakościowe oferowanego towaru określone w Załączniku nr 1 do SWZ w punktach oznaczonych indeksem DT. Zaleca się, aby w/w dokumenty potwierdzające zgodność z wymaganiami określonymi przez Zamawiającego były odpowiednio oznaczone, tj. Wykonawcy powinni oznaczyć plik/dokument, której pozycji Opisu przedmiotu zamówienia – „Wymagane minimalne parametry jakościowe” on dotyczy; 2) wydruk ze strony https://www.cpubenchmark.net/cpu_list.php, z dnia sporządzenia przez Wykonawcę dokumentu stanowiącego ofertę, potwierdzający, że zaoferowane w urządzeniach procesory wielordzeniowe osiągają w teście PassMark CPU Mark wynik spełniający minimalne wymagania jakościowe określone przez Zamawiającego dla stacji roboczych (pkt I.4.1, pkt II.4.1), komputera typu laptop (pkt III.3.1); 3) certyfikat ISO 9001:2015 lub inny dokument wydany przez niezależną jednostkę akredytującą, potwierdzający, że oferowane stacje robocze (pkt I.13.6, pkt II.13.6), komputera typu laptop (pkt III.14.6) oraz monitory (pkt IV.1.7.6) zostały wyprodukowane w organizacji posiadającej wdrożony system zarządzania jakością; 4) certyfikat ISO 14001 lub inny dokument równoważny wydany przez niezależną jednostkę akredytującą, potwierdzający, że oferowane stacje robocze (pkt I.13.7, pkt II.13.7), komputera typu laptop (pkt III.14.7) oraz monitory (pkt IV.1.7.7) zostały wyprodukowane w organizacji posiadającej wdrożony system zarządzania środowiskiem; 5) oświadczenia producenta/ów potwierdzające, że serwis urządzeń będzie realizowany bezpośrednio przez producenta i/lub we współpracy z…
