Purchase of IT infrastructure for the needs of the Centre for Mental Health II

1. The subject of this order is the purchase, supply and implementation of IT infrastructure for the needs of the Centre for Mental Health located in the Provincial Independent Team of Public Health Care Plants named after Prof. Eugeniusz Wilczkowski in Gostynin, i.e. 2 server and 1 matrix units. The implementation will include the assembly of equipment at the location indicated by the contracting authority, initialisation of equipment including the granting of IP addresses to managment interfaces, connection of the delivered servers to the matrix and the ordering LAN network, configuration of the matrix resources consisting of the provision of disk resources to the delivered servers, and preparation of post-implementation documentation containing: list of assigned addresses, established users, passwords and accounts. 2. The subject of the contract is carried out on the basis of the Grant Agreement, financed by the budget of the Ministry of Health as part of the task of “Purchase of IT infrastructure for the needs of the Centre for Mental Health” of the Provincial Independent Public Welfare Plant in Gostynin. 3. The subject of the contract referred to in paragraph 1 must be: 1) a factory new, 2) produced not earlier than 6 months before the date of delivery to the contracting authority and derived from the authorised distribution channel of the manufacturer – it concerns servers, and the matrix must be manufactured not earlier than 9 months before the date of delivery to the contracting authority and come from the authorised distribution channel of the producer, 3) covered by the manufacturer's service in the Republic of Poland, 4) be included in the list of Windows Server Catalog and have the status of “Certified for Windows” for Windows Server 2022, Windows Server 2025, 5) originally packed, not bearing the characteristics of use, 6) undamaged, 7) professionally delivered, with high quality, aesthetics and durability of performance, 8) complete and in full, after implementation ready for use, without any additional purchases, 9) consistent with the technical parameters specified in the description of the subject of the contract, which is Annex 1a to the SWZ. 4. The Contractor shall carry out the subject-matter of the contract on the terms and conditions described in the SWI and in the draft provisions of the contract constituting Annex No 2 to the SWI, the contractor's tender form being Annexes No 1 to this SWZ/Annex No 1 to the contract (integrative part of the contract), the description of the subject-matter of the contract – the technical characteristics of the computer equipment – which are Annex 1a to this SWZ and other documents affecting the proper execution of the subject-matter of this contract. 5. The range covered by the contract must have the required legal certificates, certificates, certificates of entry into service/declarations of conformity or other documentation confirming that the equipment offered is authorised on the Polish market. 6. The Contractor is obliged to grant a guarantee on the subject of the contract delivered for a minimum period of 60 months, with implementation of the 9x5 NBD onsite mode, counting from the date on which the Contracting Authority signed the protocol of the object of the contract without reservation. 7. The contractor selected as a result of this procedure will deliver the subject of the order to the seat of the Provincial Independent Public Care Plant of Prof. Eugeniusz Wilczkowski in Gostynin located at: ul. Zalesie 1, 09 – 500 Gostynin woj. mazowieckie, where its reception and implementation will take place.
