1. The offer and its attachments that the Contractor applying for the order is obliged to submit: a) Offer form – according to Annex No. 2 to the SWZ - including, among other things, a price calculation. b) Description of the minimum technical parameters required by the Contracting Authority – Annex No. 3 to the SWZ. c) An excerpt or information from the National Court Register, the Central Register and Information on Economic Activity or another relevant register, to confirm that the person acting on behalf of the Contractor is authorized to represent them. The Contractor is not required to submit the above-mentioned documents if the Contracting Authority can obtain them from free and publicly accessible databases, provided that the Contractor indicated in the offer form the data enabling access to these documents. d) A current statement as of the date of submission of the offer, serving as preliminary confirmation that the Contractor is not subject to exclusion and meets the participation conditions in the proceedings. The Contractor is obliged to submit the statement in original form as a Single European Procurement Document (JEDZ), which is Annex No. 4 to the SWZ, in the form of an electronic document, signed with a qualified electronic signature. When completing the JEDZ, the Contractor should also fill in part III - grounds for exclusion, in section D - Other grounds for exclusion that may be provided for in the national law of the member state of the contracting authority or contracting party concerning the exclusion grounds specified in Article 7(1) of the Act of April 13, 2022, on special solutions for counteracting support for aggression against Ukraine and serving the defense of national security as mentioned in Chapter 8, section 2 of the SWZ. In the case of an offer submitted by Contractors jointly applying for the order, the requirement to submit a Single European Procurement Document applies to each of the Contractors jointly applying for the order. The JEDZ concerning each of the Contractors jointly applying for the order must be submitted in the form of an electronic document, signed with a qualified electronic signature of the Contractor it pertains to. These documents confirm the absence of grounds for exclusion from the proceedings. A Contractor intending to entrust the execution of part of the order to Subcontractors, in order to demonstrate the absence of grounds for exclusion concerning them from participation in the proceedings, submits a JEDZ for each of the Subcontractors. e) The Contractor's statement regarding the grounds for exclusion from Article 5k of Council Regulation (EU) No 833/2014 of July 31, 2014, and from Article 7 of the Act of April 13, 2022, on special solutions for counteracting support for aggression against Ukraine and serving the protection of national security – according to Annex No. 5 to the SWZ. If the Contractors apply jointly for the order – each Contractor should submit the document. f) if applicable, the offer should include: • Power of attorney authorizing the submission of the offer (authorization to represent the Contractor) - if a person acts on behalf of the Contractor and their authority to represent them does not result from documents defining the legal status of the Contractor; • Power of attorney for the person acting on behalf of the Contractors jointly applying for the public procurement – applies to offers submitted by Contractors jointly applying for the order; • Power of attorney for the person acting on behalf of the entity providing resources. g) If applicable: In cases referred to in Article 117(2) and (3) of the Public Procurement Law, Contractors applying jointly for the award of a contract attach to the offer a statement referred to in Article 117(4) of the Public Procurement Law, indicating which deliveries will be executed by individual Contractors (Annex No. 9 to the SWZ). h) Relevant evidence: - Manufacturer's catalog card or technical description o…
