Do oferty Wykonawca jest zobowiązany złożyć dla danej części zamówienia: 1) Formularz ofertowy. 2) Formularz cenowy, który ma charakter pomocniczy. 3) Oświadczenie o niepodleganiu wykluczeniu w postępowaniu dla Wykonawcy w zakresie wskazanym przez Zamawiającego. W przypadku Wykonawców wspólnie ubiegających się o udzielenie zamówienia, oświadczenie, o którym mowa powyżej, składa każdy z Wykonawców.
