Individual Teletherapy (ITT)

1. Absence of mandatory exclusion grounds according to § 123 para 1, 4 GWB (self-declaration in the form "Self-declaration on suitability" required); If there is a compelling reason for exclusion, meaningful documents for self-cleaning according to § 125 GWB must be submitted; No optional grounds for exclusion according to § 124 para. 1 GWB (self-declaration in the form "Self-declaration on eligibility" required); If there is an optional reason for exclusion, meaningful documents for self-cleaning according to § 125 GWB must be submitted; Declaration that the company of the applicant does not comply with the requirements set out in Article 5 k paragraph 1 of Regulation (EU) No 833/2014 as amended by Art. 1 point. 23 of Council Regulation (EU) 2022/576 of 8 March 2022. on restrictive measures in view of Russia’s actions destabilising the situation in Ukraine, includes persons or undertakings identified as having a link with Russia within the meaning of the provision and a declaration that the undertakings involved in the contract as subcontractors, suppliers or undertakings whose capacities are used in connection with the provision of the proof of suitability, representing more than 10 % of the value of the contract, also do not belong to the category of persons referred to in the provision with a link with Russia within the meaning of the provision. Declaration that the applicant’s company has not been fined at least EUR 2,500 for a breach pursuant to § 23 AEntG and § 21 MiLoG or has been fined at least one hundred and seventy-five thousand euros for a breach pursuant to § 22 LkSG and no current breach and no pending penalty notice against the applicant’s company or the responsible person(s) pursuant to § 98c para. 1 AufenthG or according to § 21 SchwArbG (self-declaration in the form "Self-declaration on suitability" required). 5. entry in the professional or commercial register or equivalent certificate of an authority of the applicant's country of origin or provenance (self-declaration required in the form "Self-declaration of suitability"); Membership of a professional association or equivalent certificate from an authority of the applicant's country of origin or provenance (self-declaration required in the form "Self-declaration of suitability").

Total turnover of the company in the last three financial years and turnover of the company insofar as it concerns services comparable to the service to be awarded (ITT), including the share of services performed jointly with other companies.

Self-declaration on liability insurance, that a currently valid liability insurance or a comparable market-standard insurance with a liability coverage amount of at least 3.0 million. Euro for personal injury and 1.0 million Euro for property and property damage per insurance year and claim. Or, if there is no corresponding insurance cover, an irrevocable and unconditional self-declaration that, in the case of a contract, a corresponding insurance with a sufficient amount of cover is concluded and a corresponding proof is presented before the award of the contract.

Self-declaration that a membership exists in a professional association or justification that there is no membership.

Minimum Requirement: Proof of experience with the use of the software to carry out digital therapy and telerehabilitation in clinic or rehabilitation clinic in the last 3 years.
