Self-declaration on the lack of Russia-related services (with the offer; by self-declaration): A self-declaration of the existence or absence of grounds for exclusion within the meaning of Regulation (EU) 2022/576 (Russia-related).

Legal reasons for exclusion (with the offer; by means of self-declaration): A declaration must be made about the existence or absence of grounds for exclusion in the sense of §§ 123, 124 GWB (serious misconduct that calls into question the reliability as an applicant) as well as about self-cleaning measures in accordance with § 125 GWB (in Form 124).

Insolvency and liquditation (with the offer; by means of self-declaration): A declaration (in Form 124) on the existence of insolvency proceedings and liquidation must be made.

Annual turnover Supply and service (with the offer; by self-declaration): it is the turnover of the undertaking for the last three financial years in so far as it relates to services comparable to the service to be awarded; including the share of services performed jointly with other undertakings.

Subcontractor assignment (with the offer; by self-declaration): It must be stated (in Form CSX 51, VVB 233 or subcontractor list) whether parts of the contract are to be provided by subcontractors. Where this is the case, the performance ranges and the names of subcontractors shall be specified.

Personnel status Supply and service (On request of the awarding agency; by self-declaration): A proof of the average annual number of staff in the last three years shall be provided. The person designated to lead the project shall be appointed.

References Supply and service (On request of the awarding authority); By self-declaration: Three references to comparable services provided in the last three calendar years shall be provided. The supporting documents must contain information on: contact person; the type of service performed; order amount; Execution period

Registration Supply and service (with the offer; by means of a third-party declaration): - A declaration (in Form 124) about the obligation to register in a professional register must be made. A demonstration of this shall be provided on request.

Certificates of safety Supply and service (On request of the awarding authority; by means of third-party declaration): Proof of the proper tax and social security contributions must be provided and the certificate of safety of the tax office or the certificate in tax matters must be presented.
