Construction and implementation of the International and National Information Exchange System

Implementation within three years before the expiry of the time limit for the submission of tenders, and if the period of business is shorter, then during that period: at least 2 contracts consisting in the design, execution and implementation of the electronic system for electronic exchange of information and integration with external systems with a value of at least PLN 5 000 000,00 gross each. The contracting authority shall, before selecting the most favourable tender, require the contractor whose tender has been the highest rated to submit, within a specified period of time, not less than 10 days, up to the date of the submission of the following personal evidence to demonstrate compliance with the conditions for participation in the procedure: a list of deliveries carried out, confirming compliance with the condition laid down in Chapter VIII(1) of the SAA, and, in the case of interim or continuous benefits also made, during the last 3 years before the deadline for the submission of tenders, and, if the period of business is shorter, during that period, with the indication of their value, the subject matter, the dates of performance and the entities for which the supplies have been made, and with the addition of evidence indicating whether those supplies have been carried out or are carried out duly.
