Przystępując do postępowania Wykonawca zobowiązany jest złożyć:

  1. Wypełniony i podpisany: formularz oferty – zgodnie z załącznikiem nr 1 do SWZ
  2. Wypełniony i podpisany: formularz asortymentowo-cenowy – zgodnie z załącznikiem nr 1a do SWZ.
  3. Wypełnione i podpisane: Zestawienie wymagań technicznych i jakościowych /Formularz spełniania wymagań technicznych i jakościowych– zgodnie z załącznikiem nr 2.1-2.5 do SWZ (dotyczy części, o które ubiega się wykonawca).
  4. Potwierdzenie umocowania do działania w imieniu wykonawcy, podwykonawcy lub podmiotu udostępniającego zasoby:
  1. W celu potwierdzenia, że osoba działająca w imieniu wykonawcy, podwykonawcy lub podmiotu udostępniającego zasoby jest umocowana do jego reprezentowania, zamawiający żąda od wykonawcy odpisu lub informacji z Krajowego Rejestru Sądowego, Centralnej Ewidencji i Informacji o Działalności Gospodarczej lub innego właściwego rejestru.
  2. Wykonawca nie jest zobowiązany do złożenia dokumentów, o których mowa w pkt. 1, jeżeli zamawiający może je uzyskać za pomocą bezpłatnych i ogólnodostępnych baz danych, o ile wykonawca wskazał dane umożliwiające dostęp do tych dokumentów.
  3. Jeżeli w imieniu wykonawcy działa osoba, której umocowanie do jego reprezentowania nie wynika z dokumentów, o których mowa w pkt. 1, zamawiający żąda od wykonawcy pełnomocnictwa lub innego dokumentu potwierdzającego umocowanie do reprezentowania wykonawcy.
  4. Pełnomocnictwo do reprezentowania wykonawców wspólnie ubiegających się o udzielenie zamówienia w postępowaniu o udzielenie zamówienia albo do reprezentowania ich w postępowaniu i zawarcia umowy w sprawie zamówienia publicznego (jeżeli dotyczy). Informujemy, że dokumenty wyszczególnione w ust. 1, 2, 3 stanowią treść oferty i nie podlegają uzupełnieniu.
  5. Zamawiający wezwie Wykonawcę, którego oferta została najwyżej oceniona, do złożenia w wyznaczonym terminie, nie krótszym niż 5 dni od dnia wezwania, oświadczenia Wykonawcy o aktualności informacji zawartych w oświadczeniu, o którym mowa w Rozdziale 10 ust. 1 pkt. 1), w zakresie podstaw wykluczenia z postępowania wskazanych w Rozdziale 9, (zgodnie z Załącznikiem nr 5 do SWZ).
