Development of e-health services along with the development of IT systems and the development of IT infrastructure in Hrubieszów

(a) in the scope of the contractor's experience - this condition will be met if the Contractor demonstrates that: i. in the last 3 years before the deadline for the submission of tenders, and if the duration of the business is shorter - during that period, he has completed 2 or more orders to supply the HiS-class IT system with the supply of IT infrastructure (including at least one order including the delivery of: servers, matrices) with a value of not less than PLN 2,000,000.00 gross each, and ii. in the last 3 years before the deadline for the submission of tenders, and if the duration of the business activity is shorter - during that period, he has made at least one delivery, covering the delivery of equipment and software, including at least the servers and matrices and server software and computers and monitors, UPSs with a value of at least PLN 2,000,000.00.00 gross and shall attach evidence that the specified deliveries have been duly executed. Attention! The Contracting Authority shall not allow the summation of the value of services/supply under different contracts/orders/contracts, i.e. The Contracting Authority shall not allow the aggregation of several smaller orders whose value is less than the indicated value of one service. Attention! In the case of Contractors jointly applying for a contract and in the case of relying on the resources of third parties The Contracting Authority shall permit the cumulative demonstration of the experience condition. At the same time, the Contracting Authority reserves that part of the contract for which the entity demonstrates experience must be performed by that entity in person. Attention! Where a third party that makes available the potential of an economic operator has executed a contract to which the services/deliveries referred to above covered jointly with another entity not applying for a contract, the contracting entity shall require that the third entity making available the potential of the economic operator actually participate in the implementation of the above services. Personal evidence concerning the confirmation of compliance with the conditions for participation in the proceedings requested by the Contracting Authority, the offer of which has been assessed at most (within the time limit set by the Contracting Authority, not less than 10 days). The list of deliveries (according to the formula set out in Annex 9 to the SWZ) carried out: (a) during the last 3 years before the deadline for the submission of tenders, and if the period of business is shorter - during that period, he has carried out 2 or more contracts involving the supply of the IT infrastructure (including at least one contract covering the supply of: servers, matrices) with a value of not less than PLN 2,000,000,00 gross each, and (b) during the last 3 years before the deadline for the submission of tenders, and if the period of business is shorter - during that period, he has carried out at least one delivery, including the delivery of hardware and software, to which at least the servers and matrices and server software, and computers and monitors, UPSs with a value of at least PLN 2,000,000,00 gross, together with the addition of evidence to determine whether these supplies have been duly completed, with the evidence referred to, or other documents drawn up by the entity for which they were supplied, and if the contractor is unable to obtain the relevant documents.

(b) concerning persons addressed by the contractor for the performance of the contract - this condition will be met if the Contractor refers: and. Project Manager (minimum 1 person) qualified with valid certificates: ‘PRINCE2’ or IPMA or equivalent; who has completed at least 2 projects with a budget equal to or more than PLN 2 000 000,00 gross during the last 5 years before the deadline for submission of tenders; ii. at least 1 person who holds a certified Information Systems Security Professional (CISSP) security certificate or equivalent, iii. at least 1 person holding a certificate of security of the Certified Ethical Hacker (CEH) or equivalent, i.e. at least 1 person holding a Certified Internal Auditor (CIA) certificate or equivalent.
