- The subject of the procedure is the award of an order for the Contractor to provide services to the Contracting Authority, together with the authorial supervision (maintenance) to ensure the day-to-day operation of the Application Software, i.e.:
- of the Eskulap IT system of Nexus Polska Sp. z o.o. with its registered office in Poznań, ul. Szyperska 14, 61-754 Poznań and
- of the Impulse EVO IT system of Forterro Polska Sp. z o.o. with its registered office in Katowice, al. Valenty Roździeńskiego 188h, 40-203 Katowice, held by the Ordering Authority. held by the Contracting Authority.
- The subject-matter of the contract shall be carried out in accordance with the model contract, which shall be annexed No 4 to the EGM and on the basis of the following annexes to the contract:
- List of applications covered by the service services and their Manufacturers and description of the infrastructure of the Contracting Authority
- Annex to contract No 1;
- Scope of services provided
- Annex to Agreement No 2;
- Security contacts and procedures
- Annex to Agreement No 3;
- Agreement entrusting the processing of personal data
- Annex to Agreement No 4;
- The Contracting Authority’s information obligation clause
- Annex to Agreement No 5. All the above Annexes to the Agreement shall also be annexed to this UNSC as an integral part thereof.
- The Contractor shall provide services subject to the contract with particular care and in accordance with the industry standards adopted for similar services, taking into account the specific characteristics of the Contracting Authority's activities.

Zdolność techniczna lub zawodowa: Warunek zostanie spełniony jeżeli Wykonawca wykaże, że wykonał, a w przypadku świadczeń powtarzających się lub ciągłych również wykonuje, w okresie ostatnich 3 lat, a jeżeli okres prowadzenia działalności jest krótszy – w tym okresie, jedną usługę polegającą na wykonaniu usług serwisowania systemu informatycznego Eskulap i systemu informatycznego Impuls EVO w ramach jednej umowy, o wartości nie mniejszej niż 200 tys. zł brutto, wraz z podaniem ich wartości, przedmiotu, dat wykonania i podmiotów, na rzecz których usługi zostały wykonane lub są wykonywane, oraz załączeniem dowodów określających, czy te usługi zostały wykonane lub są wykonywane należycie.
