Undertakings must provide either the completed “Self-declaration of eligibility for services” or a Single European Self-Declaration (EEE) with the offer as proof of eligibility for the service to be awarded. If the offer is shortlisted, companies have to submit the company-related documents required in the "List of documents to be submitted in the procurement procedure for services" to the customer at the time specified by him on a separate request. If other companies are used, the completed "Self-declaration of suitability for services" and the company-related documents must also be submitted for them on separate request. Evidence can also be obtained by entering an official register (e.g. the PQ register established by the Chamber of Commerce and Industry) or by submitting a certificate in accordance with European certification standards. Certificates not drawn up in German must be accompanied by a translation into German. The form “Self-declaration on the suitability of services” is attached to the procurement document.

Suitability for professional practice, economic and financial capacity and technical and professional capacity self-declarations for the following suitability requirements: > a statement of the company's turnover in the last three financial years, insofar as it relates to services comparable to the service to be awarded, including the share of services performed together with other companies > Declaration that similar services have been performed in the last three years > Declaration that the employees required to perform the services are available > self-declaration on registration in a professional register > self-declaration that no insolvency proceedings or similar legally regulated proceedings have not been requested or opened, a request for opening has not been rejected due to lack of assets and the company is not in liquidation or an insolvency plan has been validly confirmed and is submitted on request > Self-declaration that no exclusion grounds am. § 123 and § 124 GWB are available > self-declaration that the company has not been punished in the last two years by a custodial sentence of more than three months or a fine of more than 90 daily rates or a fine of more than 2,500 euros > self-declaration that although there is a reason for exclusion for the company according to § 123 GWB, measures have been taken for the company for self-cleaning, by which the company was restored reliability > Self-declaration that no exclusion requirements according to § 22 Abs. 1 S. 1 of the Supply Chain Due Diligence Act (LkSG) and the company is not subject to a fine acc. § 22 Abs. 2 LkSG has been occupied > self-declaration of proper fulfilment for the payment of taxes, duties and contributions to the statutory social security, insofar as they are subject to the obligation to pay contributions > Information on membership of a professional association The following order-specific self-declarations must be submitted with the offer by the bidder or the other companies employed: > for used licenses: Appendix - self-declaration regarding suitable proof of exhaustion requirements and declaration of exemption for used software > The company-related documents (which must be submitted at the request of the client - upon request) include the following information: > List of three references (comparable services) from the last three years with at least the following information: contact person; the type of service performed; order amount; Execution period > Declaration on the number of employees employed annually on average in the last three years and designation of persons intended for management > Certificate of registration in the professional register (if the obligation to register exists) > legally confirmed insolvency plan (if a declaration of the existence of such insolvency plan has been provided) > Certificate of safety of the tax office or certificate in tax matters, if the tax office issues such a certificate The following order-specific proofs must be submitted on request by the bidder or the other companies employed: > for used licenses: original documents, declarations and proofs on the chain of rights [see point 3 - 2. - in self-declaration for used licenses]
