What the buyer wants
The subject of this tender procedure is: Purchase of the Assistant Doctor integrated with the system of His with the Clinic, which is referred to as ‘Software’ according to the type of product and quantity specified in the ‘Production and Price Formation’, which is Annex No. 2 to the SWZ and in accordance with the ‘Formulator Offers’, which is Annex No. 1 to the SWZ and the Description of the Order Subject, which is Annex No. 1a and Annex 1b – Warranty and Service Conditions for the Independent Public Health Care Department of the University Hospital No. 2 of Medicine in Łódź, ul. Żeromskiego 113, 90-549 Łódź, the Independent Public Health Care Plant of the Central Medical Hospital in Łódź, ul. 921-213 Łódź. In the Product-Price Form, in the corresponding columns, the quantities to be ordered by the Independent Public Health Care Department of the University of Łódź Medical Hospital No. 2, the Independent Public Health Care Department of the University of Łódź Medical Hospital No. 1 and the Independent Public Health Care Department of the Central Medical University of Łódź. Each Ordering Party shall sign a separate contract for the award of the contract in respect of the quantities specified in the Product-Price Form for the hospital concerned.
Przedmiotem zamówienia niniejszego postępowania przetargowego jest: Zakup Asystenta Lekarza zintegrowanego z systemem HIS z Clininet zwanych dalej „Oprogramowaniem” zgodnie z rodzajem asortymentu i ilościami określonymi w „Formularzu asortymentowo-cenowym” stanowiącym załącznik nr 2 do SWZ i zgodnie z „Formularzem Oferty”, stanowiącym załącznik nr 1 do SWZ oraz Opisem Przedmiotu Zamówienia, stanowiącym załącznik nr 1.a i załącznikiem 1 b – Warunki Gwarancji i serwisu dla Samodzielnego Publicznego Zakładu Opieki Zdrowotnej Uniwersyteckiego Szpitala Klinicznego nr 2 Uniwersytetu Medycznego w Łodzi, ul. Żeromskiego 113, 90-549 Łódź, Samodzielnego Publicznego Zakładu Opieki Zdrowotnej Uniwersyteckiego Szpitala Klinicznego Nr 1 im. Norberta Barlickiego Uniwersytetu Medycznego w Łodzi, ul. Kopcińskiego 22, 90-153 Łódź oraz Samodzielnego Publicznego Zakładu Opieki Zdrowotnej Centralnego Szpitala Klinicznego Uniwersytetu Medycznego w Łodzi ul. Pomorska 251, 92-213 Łódź. W Formularzu asortymentowo-cenowym, w odpowiednich kolumnach, są wskazane ilości, jakie zostaną zamówione przez Samodzielny Publiczny Zakład Opieki Zdrowotnej Uniwersytecki Szpital Kliniczny nr 2 Uniwersytetu Medycznego w Łodzi, Samodzielny Publiczny Zakład Opieki Zdrowotnej Uniwersyteckiego Szpitala Klinicznego nr 1 im. Norberta Barlickiego Uniwersytetu Medycznego w Łodzi oraz Samodzielny Publiczny Zakład Opieki Zdrowotnej Centralny Szpital Kliniczny Uniwersytetu Medycznego w Łodzi. Każdy z Zamawiających podpisuje odrębną umowę w sprawie udzielenia zamówienia w zakresie ilości określonych w Formularzu asortymentowo-cenowym dla danego szpitala. Realizacja przedmiotu postępowania jest współfinansowana z środków Agencji Badań Medycznych w ramach konkursu nr ABM/2023/2 Tworzenie i rozwój Regionalnych Centrów Medycyny Cyfrowej. Nazwa projektu: Medical University of Lodz – Digital Medicine Center ( MULDiMediC ).
