Wraz z ofertą Wykonawca jest zobowiązany załączyć: 1) Przygotowany (sporządzony przez Wykonawcę) i podpisany przez Wykonawcę opis oferowanego asortymenty (z podaniem producenta i parametrów technicznych). Opis stanowi treść oferty poprzez skonkretyzowanie cech kryjących się pod wskazaną nazwą oferowanego przez Wykonawcę oprogramowania. Opis Wykonawca składa wraz z ofertą. Uwaga: Brak złożenia wraz z ofertą przygotowanego przez Wykonawcę i podpisanego przez niego opisu oferowanego przedmiotu zamówienia będzie skutkowało odrzuceniem oferty. 2) Aktualne na dzień składania ofert oświadczenie o spełnianiu warunków udziału w postępowaniu oraz o braku podstaw do wykluczenia z postępowania - zgodnie z Załącznikiem nr 2 do SWZ; 3) Pełnomocnictwo do podpisania oferty (jeżeli dotyczy); 4) Zobowiązanie innego podmiotu, o którym mowa w Rozdziale X ust. 3 SWZ (jeżeli dotyczy).
