  1. Ofertę składa się pod rygorem nieważności, zgodnie z wyborem Wykonawcy:
    1. w formie elektronicznej (oznacza to postać elektroniczną opatrzoną kwalifikowanym podpisem elektronicznym), lub
    2. w postaci elektronicznej opatrzonej podpisem zaufanym lub podpisem osobistym.
  2. Ofertę stanowi wypełniony formularz „Oferta” (zał. nr 2 do SWZ) wraz ze „Specyfikacją oferowanego przedmiotu zamówienia” (załączniki nr 5.1 - 5.3 do SWZ, odpowiednio do części).
  3. Wraz z ofertą Wykonawca jest zobowiązany złożyć za pośrednictwem Platformy:
  1. oświadczenie na podstawie art. 125 ust. 1 uPzp o niepodleganiu wykluczeniu (zał. nr 3 do SWZ) - w formie elektronicznej lub w postaci elektronicznej opatrzonej podpisem zaufanym lub osobistym;
  2. odpis lub informację z Krajowego Rejestru Sądowego, Centralnej Ewidencji i Informacji o Działalności Gospodarczej lub innego właściwego rejestru (w celu potwierdzenia uprawnienia do reprezentowania podmiotu) - chyba, że Zamawiający może je uzyskać za pomocą bezpłatnych i ogólnodostępnych baz danych, a Wykonawca wskazał w ofercie dane umożliwiające dostęp do tych dokumentów (np. nr KRS, NIP);
  3. pełnomocnictwo lub inny dokument potwierdzający umocowanie do reprezentowania Wykonawcy, chyba, że umocowanie do reprezentacji wynika z dokumentów wskazanych w pkt 2) powyżej;
  4. w przypadku Wykonawców wspólnie ubiegających się o udzielenie zamówienia: pełnomocnictwo potwierdzające umocowanie do reprezentowania wszystkich Wykonawców wspólnie ubiegających się o udzielenie zamówienia, o którym mowa w Rozdz. IX ust. 3; oświadczenie z zał. nr 3 składa się oddzielnie dla każdego wykonawcy.
