The subject of the contract is the provision of fibre/digital link rental service for the ZUS Branch in Zielona Góra, in relation to: Zielona Góra, ul. Wyspiańskiego 15B - Zielona Góra, ul. Kupiecka 65. A detailed description of the subject matter of the contract is given in Annex No 10 to the EGM, and the conditions for the performance of the contract include the draft provisions of the contract constituting Annex No 11 to the EGM. For all parts of the contract: The contractor shall comply with the condition if it demonstrates that during the last 3 years before the deadline for the submission of tenders and if the period of business is shorter, during that period he has duly or continuously performed a service of lease of a cable line between at least 2 buildings for at least 12 consecutive months in a single contract. Notes for all parts of the order: NOTE No 1: Where the Contractor uses the experience acquired on the basis of the services provided under the consortium, the Contractor may rely on the provision of such a service if it has actually and specifically participated in its implementation. NOTE 2: With regard to the conditions relating to experience, Contractors jointly applying for the award of a contract may rely on the capacity of those Contractors who have provided the services to which those capacities are required. NOTE 3: In the case of Contractors jointly applying for a contract, the condition of participation in the procedure shall be deemed to be fulfilled if the condition is fulfilled in full by at least one of the Contractors jointly applying for a contract. NOTE 4: In cases where the Contractor has the capacity of other entities to confirm that the condition of participation is met, the condition must in its entirety meet the minimum of one of these entities. NOTE 5: The Contracting Authority allows the indication of the same service to fulfil the condition of participation in the procedure in the various parts of the contract for which the Contracting Authority submits the offer. To confirm compliance with the condition of participation in the proceedings, the Contractor shall submit a list of the services performed and, in the case of recurrent or continuous services, also carried out, over the last 3 years counted backwards from the date on which the time limit for the submission of tenders expires and, if the period of business is shorter, during that period, together with the dates of execution/execution and the entities for which the services have been carried out or are carried out in accordance with Annex 5 to the SAA and the addition of evidence indicating whether those services have been performed or duly performed; Proof that the services have been performed or are duly performed shall be: references or other documents drawn up by the entity for which the services have been carried out, and in the case of recurrent or continuous benefits, and if the Contractor for reasons beyond his control is unable to obtain such documents, the Contractor's statement; in the case of services which are repeated or continued to be performed, or other supporting documents for their proper performance, shall be issued within the last 3 months of the date on which the time limit for the submission of tenders expires.
