COMPLETED VITAL FRONT FOR NEURO-ECHYLITICAL DISEASES

The purpose of this Convention is to implement a comprehensive national-wide project concerning the design, development, pilot implementation and evaluation of a modern model of digital care for people with neurodegenerative diseases, with an initial focus on dementia. The project aims to create a single and coherent framework for care services, which uses digital tools to enhance the quality, continuity and effectiveness of care, both at clinical and organisational level. The contract concerns a multidimensional project, in which organizational, clinical, operational and technological interventions are combined. Individual actions are not self-contained, but constitute a single care system designed to support the integrated treatment of the needs of patients and their carers, as well as the systematic support of health professionals.
