  2. Wypełnione i podpisane Oświadczenie Wykonawcy składane na podstawie art. 125 ust. 1 ustawy – załącznik nr 3 do SWZ.
  3. Projekt umowy o udzielenie przedmiotowego zamówienia, uwzględniający zapisy określone w Części E pkt I.
  4. Pełnomocnictwo w przypadku gdy osoba lub osoby podpisujące ofertę działają na podstawie pełnomocnictwa i/lub pełnomocnictwo do reprezentowania Wykonawcy/Wykonawców w postępowaniu o udzielenie zamówienia. Forma pełnomocnictwa została określona w pkt IX.6 niniejszej części. 5) Zobowiązanie podmiotów do oddania do dyspozycji Wykonawcy niezbędnych zasobów na potrzeby realizacji przedmiotowego zamówienia – załącznik nr 6 do SWZ – jeżeli dotyczy.
  5. Oświadczenie Wykonawcy składane na podstawie art. 125 ust. 5 ustawy – załącznik nr 5 do SWZ – jeżeli dotyczy.
