What the buyer wants
- The subject of the contract shall be the delivery of the monitor (1 pcs) referred to in Annex 2 to the SWZ, hereinafter referred to as ‘the equipment’. The equipment offered must be brand-new and unused, coming from the current production (year of production: not earlier than 2025), have appropriate certificates allowing it to be sold and used in Poland. The contractor is obliged to provide free warranty service for the equipment.
- The scope of the order includes: purchase and delivery of equipment to the premises of the Ordering Party, including unloading, transfer to the place designated by the Ordering Party. 3.The Contracting Authority shall require that the provision offered, including all ICT products, ICT services and ICT processes used for the performance of the contract, both as principal elements and components, elements of the service environment or accompanying solutions, does not include: - ICT products, ICT services or ICT processes identified in the recommendation referred to in Article 33(4) of the Law of 5 July 2018 on the national cybersecurity system, stating their negative impact on the essential interest of the security of the State;- ICT product of which the type is defined in the decision to recognise the supplier as a high risk provider referred to in Article 67b(15) of the Law of 5 July 2018 on the national cybersecurity system, or ICT services or ICT processes specified in that decision.Failure to comply with this requirement will result in the rejection of the tender on the basis of Article 226(1), point 17 or point 19, respectively, of the Public Procurement Act;

- Wykonawca, który polega na zdolnościach lub sytuacji podmiotów udostępniających zasoby, składa wraz z ofertą: Zobowiązanie podmiotu udostępniającego zasoby do oddania mu do dyspozycji niezbędnych zasobów na potrzeby realizacji zamówienia lub inny podmiotowy środek dowodowy potwierdzający, że Wykonawca realizując zamówienie, będzie dysponował niezbędnymi zasobami tych podmiotów. Wzór pisemnego zobowiązania stanowi załącznik 1D do SWZ.Zobowiązanie, o którym mowa w ust. 4 pkt. 1 składane jest w oryginale w formie elektronicznej, w postaci elektronicznej z podpisem zaufanym lub osobistym albo kopia w postaci cyfrowego odwzorowania dokumentu papierowego, poświadczona za zgodność z oryginałem przez podmiot udostępniający zasoby podpisem kwalifikowanym, zaufanym lub osobistym lub przez notariusza podpisem kwalifikowanym. Dokument składany wraz z ofertą (jeżeli dotyczy); .
- W celu potwierdzenia, że osoba działająca w imieniu Wykonawcy jest umocowana do jego reprezentowania, Zamawiający żąda od Wykonawcy: odpisu lub informacji z Krajowego Rejestru Sądowego, Centralnej Ewidencji i Informacji o Działalności Gospodarczej lub innego właściwego rejestru. . 4.Jeżeli w imieniu Wykonawcy działa osoba, której umocowanie do jego reprezentowania nie wynika z dokumentów, o których mowa w pkt 1, Zamawiający żąda od Wykonawcy złożenia: pełnomocnictwa lub innego dokumentu potwierdzającego umocowanie do reprezentowania Wykonawcy.Wymóg powyższy ma zastosowanie odpowiednio do osoby działającej w imieniu Wykonawców wspólnie ubiegających się o udzielenie zamówienia publicznego.Dokumenty, o których mowa w pkt. 3 i 4 składane są wraz z ofertą w oryginale w formie elektronicznej, w postaci elektronicznej z podpisem zaufanym lub osobistym albo kopii w postaci cyfrowego odwzorowania dokumentu papierowego, poświadczonej za zgodność z oryginałem odpowiednio przez Wykonawcę (w przypadku dokumentów z pkt 3) lub mocodawcę (w przypadku dokumentów z pkt 4) podpisem kwalifikowanym, zaufanym lub osobistym lub notariusza podpisem kwalifikowanym
