Wykaz usług w okresie ostatnich trzech lat przed upływem terminu składania ofert albo wniosków o dopuszczenie do udziału w postępowaniu, a jeżeli okres prowadzenia działalności jest krótszy – w tym okresie, wraz z podaniem ich wartości, przedmiotu, dat wykonania i podmiotów, na rzecz których usługi zostały wykonane, oraz załączeniem dowodów określających czy te usługi zostały wykonane lub są wykonywane należycie, przy czym dowodami o których mowa, są referencje lub inne dokumenty wystawione przez podmiot na rzecz którego usługi były wykonywane, a jeżeli z uzasadnionej przyczyny o obiektywnym charakterze wykonawca nie jest w stanie uzyskać tych dokumentów – oświadczenie wykonawcy.Usługi odpowiadające swoim rodzajem i zakresem usługom wskazanym w przedmiocie zamówienia, min.: - wykonania należycie co najmniej jednej usługi polegającej na opracowaniu lub aktualizacji ewidencji dróg publicznych wraz z dostarczeniem danych w formie elektronicznej.
