Maintenance of devices and networks related to DIP boards in the City of Krakow

1) He has insurance against civil liability in the scope of his business related to the subject of the contract for a guarantee amount of at least PLN 100 000,00. To be confirmed, the Contractor shall submit: Document confirming that the Contractor is insured against civil liability in the scope of the activity related to the subject-matter of the contract, indicating the total guarantee of that insurance, at least PLN 100 000,00

1) It has knowledge and experience, i.e. over the last 3 years before the deadline for the submission of tenders, and if the duration of its activities is shorter, it has carried out, and in the case of recurrent or continuous services, it shall:

• at least one service consisting of the supervision and maintenance of devices and networks related to DIP boards in a city with at least 200 plates of this type.To be confirmed, the Contractor shall submit: a list of services performed and, in the case of services which are either repeated or continuous, also performed over the last 3 years, and if the duration of the activity is shorter, during that period, with the indication of their value, the subject matter, the dates of performance and the entities for which the services have been performed and the addition of evidence indicating whether those services have been carried out or are carried out properly, with the evidence referred to, references or other documents drawn up by the entity for which the services have been carried out, and in the case of services which are repeated or continuous, and if the Contractor, for reasons beyond his control, is unable to obtain such documents, the Contractor's statement, in the case of services which are repeated or continued, references or other documents supporting their proper performance should be issued within the last 3 months, in accordance with Annex 4 to the SWZ, Note!If the Contractor cites experience in the provision of services performed jointly with other Contractors, then only those services in which the Contractor directly participated must be included in the list of services.

1) They have the ability to perform the contract, i.e. persons with an education and professional qualifications:

• at least one person holding the G1 Group qualification certificate for operation (E) and supervision (D) of equipment, installations and electricity networks up to 1 kV.To be confirmed, the Contractor shall submit: a list of the persons sent by the Contractor to carry out the public contract, in particular those responsible for the provision of services, together with information on their professional qualifications, powers, experience and education necessary for the performance of the public contract, as well as on the extent of their activities and on the basis of their disposal, as set out in Annex 13 to the SWI
