Contract award may be requested by Contractors who: 1. They have the power to conduct a specific economic or professional activity, i.e.: 1) have an entry in the register of telecommunications operators referred to in Article 5(1) of the Law of 12 July 2004. Contract award may be requested by Contractors who: In the last three years before the deadline for the submission of tenders, and if the period of business is shorter - during that period, they have completed at least two contracts, each consisting in providing a telecommunications service MPLS (Multi Protocol Label Switching) with a value of not less than 2,500 000,00 (say: two million five hundred thousand) gross PLN each.

In the case of the Contractor's making an order in the course of implementation, the completed part must reach at least PLN 2,500 000,00 (in words: two million, five hundred thousand) gross PLN Note: The condition described above must be met by the Contractors jointly applying for the award of a contract which has an entry in the register of telecommunications operators referred to in Article 5(1) of the Law of 12 July 2004. In the event of a condition referred to in paragraph 3 above in accordance with Articles 117(1) and (3) and 118(2) of the Act, The Contracting Authority shall stipulate that the condition described in paragraph 3 above must be met by one of the Contractors jointly applying for the award of the contract or, where the Contractor relies on the capacity of other entities under the conditions laid down in Article 118 of the Act, by the making available of the professional capacity, and the Contractor or, as appropriate, by the making available of the services to which those capacities are required.

Personal evidence: List of contracts executed in the last three years before the deadline for the submission of tenders, and if the period of business is shorter, during that period, meeting the condition set out in Chapter X(3) of the SWZ (as set out in the Annex to the SWZ).

Each contract listed shall be accompanied by: value, subject matter, date and place of performance (dd/mm/yyyy) and entities for which the services have been carried out and with evidence indicating whether these services have been duly performed or duly performed. 1. the references or other documents drawn up by the entity for which the supplies/services/works have been carried out, and in the case of recurrent or continuous benefits they are carried out,

If the Contractor cites experience in the provision of services, performed jointly with other Contractors, the list shall concern the services in which the performance is performed The contractor directly participated, and in the case of recurrent or continuous benefits in which the performance was directly involved or participated.
