Zdolności technicznej lub zawodowej: Opis sposobu spełnienia warunku: Zamawiający określa warunek udziału w postępowaniu w zakresie zdolności technicznej lub zawodowej, o którym mowa w art. 112 ust. 2 pkt 4 w zw. z art. 116 ust. 1 ustawy Pzp. Warunek zostanie uznany za spełniony, jeżeli Wykonawca wykaże, że w okresie ostatnich 3 lat przed upływem terminu składania ofert, a jeżeli okres prowadzenia działalności jest krótszy – w tym okresie, wykonał należycie co najmniej jedną dostawę odpowiadającą swoim rodzajem dostawom stanowiącym przedmiot części zamówienia, na którą składa ofertę, o wartości nie mniejszej niż:
- dla Części I – Zestawy komputerowe, laptopy i oprogramowanie – 80 000,00 zł brutto;
- dla Części II – Urządzenia drukujące i wielofunkcyjne – 3 000,00 zł brutto;
- dla Części III – Sprzęt komputerowy, sieciowy i multimedialny – 55 000,00 zł brutto;
- dla Części IV – Zestaw do zajęć EEG Biofeedback – 11 000,00 zł brutto. W przypadku składania oferty na więcej niż jedną część Wykonawca może wykazać tę samą dostawę na potwierdzenie spełniania warunku dla kilku części, jeżeli jej przedmiot obejmował dostawy odpowiadające rodzajem tym częściom, a jej wartość była co najmniej równa sumie minimalnych wartości wymaganych dla tych części. Wartości warunków ustalono na poziomie około 50% wartości poszczególnych części wynikających z przeprowadzonego szacowania, z zaokrągleniem w dół, w celu zachowania proporcjonalności warunku i nieograniczania konkurencji.

Wykonawca zobowiązany jest złożyć wraz z ofertą następujące dokumenty i oświadczenia: Formularz ofertowy – zgodnie z załącznikiem nr 1 do SWZ. Oświadczenie Wykonawcy, o którym mowa w art. 125 ust. 1 ustawy Pzp, dotyczące niepodlegania wykluczeniu z postępowania oraz spełniania warunków udziału w postępowaniu – zgodnie z załącznikiem nr 2 do SWZ.
Oświadczenie podmiotu udostępniającego zasoby dotyczące niepodlegania wykluczeniu oraz spełniania warunków udziału w postępowaniu w zakresie udostępnianych zasobów – zgodnie z załącznikiem nr 3 do SWZ, jeżeli dotyczy. Oświadczenie Wykonawców wspólnie ubiegających się o udzielenie zamówienia, o którym mowa w art. 117 ust. 4 ustawy Pzp, wskazujące, które dostawy wykonają poszczególni Wykonawcy – zgodnie z załącznikiem nr 6 do SWZ, jeżeli dotyczy.
Zobowiązanie podmiotu udostępniającego zasoby do oddania Wykonawcy do dyspozycji niezbędnych zasobów na potrzeby realizacji zamówienia lub inny dokument potwierdzający rzeczywisty dostęp do tych zasobów – zgodnie z załącznikiem nr 7 do SWZ, jeżeli dotyczy.
