- The subject of this procurement procedure is: the purchase of disk boards together with the necessary infrastructure to ensure continuity of operation of hospital information systems, meeting the description and limit parameters specified in the Annex ‘Description of the subject of the contract’ — Annex No 2 to the SWZ (This description should be read together with any changes to the content of the specifications, such as the results of the responses to the Contractors’ requests).

- Opis przedmiotu zamówienia, którego wzór stanowi Załącznik nr 2 do SWZ – wypełniony w zakresie tej części, na którą Wykonawca składa ofertę,
- Wypełnione oświadczenie, o którym mowa w art. 125 ust. 1 ustawy Pzp, stanowiące Załącznik nr 3 do SWZ:
- Wykonawca w odpowiedzi na ogłoszenie o zamówieniu dołącza do oferty oświadczenie, o którym mowa w art. 125 ust. 1 PZP w formie elektronicznej lub w postaci elektronicznej opatrzonej podpisem zaufanym lub podpisem osobistym;
- Oświadczenie, o którym mowa w art. 125 ust. 1 PZP, stanowi dowód potwierdzający brak podstaw wykluczenia z postępowania oraz spełnianie warunków udziału w postępowaniu na dzień składania ofert, tymczasowo zastępujący wymagane przez Zamawiającego podmiotowe środki dowodowe;
- W przypadku wspólnego ubiegania się o zamówienie przez Wykonawców, oświadczenie, o którym mowa w art. 125 ust. 1 PZP, składa każdy z Wykonawców. Oświadczenia te potwierdzają brak podstaw wykluczenia z postępowania oraz spełnianie warunków udziału w postępowaniu w zakresie, w jakim każdy z wykonawców wykazuje spełnianie warunków udziału w postępowaniu;
- W przypadku polegania na zdolnościach lub sytuacji podmiotów udostępniających zasoby - oświadczenie, o którym mowa w art. 125 ust. 1 PZP - oświadczenie podmiotu udostępniającego zasoby, potwierdzające brak podstaw wykluczenia tego podmiotu oraz odpowiednio spełnianie warunków udziału w postępowaniu w zakresie, w jakim Wykonawca powołuje się na jego zasoby.
- Przedmiotowe środki dowodowe, o których mowa pkt 8.
- Pełnomocnictwo:
- Gdy umocowanie osoby składającej ofertę nie wynika z dokumentów rejestrowych, wykonawca, który składa ofertę za pośrednictwem pełnomocnika, powinien dołączyć do oferty dokument pełnomocnictwa obejmujący swym zakresem umocowanie do złożenia oferty lub do złożenia oferty i podpisania umowy.
- W przypadku wykonawców ubiegających się wspólnie o udzielenie zamówienia wykonawcy zobowiązani są do ustanowienia pełnomocnika. Dokument pełnomocnictwa, z treści którego będzie wynikało umocowanie do reprezentowania w postępowaniu o udzielenie zamówienia tych wykonawców należy załączyć do oferty.
- Wykonawcy wspólnie ubiegający się o udzielenie zamówienia dołączają do oferty oświadczenie, z którego wynika, które dostawy wykonują poszczególni Wykonawcy (art. 117 ust. 4 ustawy Pzp) – sporządzone zgodnie z treścią Załącznika nr 6 do SWZ.
