SUPPLY, PROJECT MANAGEMENT OF IMPLEMENTATION, TRAINING, COMMISSIONING, SUPPORT AND MAINTENANCE OF OPERATING ROOM MANAGEMENT SOFTWARE, STERILIZATION (LOT 1) AND ENDOSCOPY (LOT 2) FOR AHSM (DURATION: 10 YEARS)

A statement concerning the overall turnover of the undertaking and the turnover of the field of activities covered by the contract covering the last three financial years available according to the date of establishment of the undertaking or the start of activities of the economic operator, provided that the information on those turnovers is available. Minimum level(s): The total turnover must reach a minimum of €60,000.00 VAT in total over the last 3 years. The turnover in the field of activity (process management software and sterilization and maintenance) must reach at least €300.00 VATC in total over the last 3 years.

Provide a list of references made in the last three years, indicating the amount, date and recipient Minimum level(s): The applicant must attach 3 certificates of performance for contracts relating to the supply, implementation and maintenance of operating unit and/or sterilization management software relating to a hospital institution:

• at least two of the three certificates must relate to the management of the operating unit;
• at least one of the three certificates must cover both the management of the operating unit and the sterilization;
• minimum amount of €100.00.00 VAT per certificate for the last 3 years;

The candidate joins with his offer the composition of his project team. Minimum level(s): The project manager will justify a minimum of 3 years of experience in managing software implementation projects in one or more hospital institutions or equivalent. The applicant will include a descriptive score from his project team as well as the CV of the proposed project leader who has demonstrated his experience. If he leaves the company, a new CV will be submitted to the contracting authority for approval.

The DUME, by which the economic operator declares that he is not in one of the exclusionary situations referred to in Articles 67 to 69 of the Law of 17 June 2016. Article 70 of the Public Procurement Act of 17 June 2016 applies. For the reasons of exclusion referred to in Article 67 of the Law, the candidate or tenderer shall indicate on his own initiative whether he has taken the corrective measures referred to in Article 70(1) at the beginning of the procedure.

Minimum level(s): The applicant will have to attach 3 performance certificates for contracts relating to the supply, implementation and maintenance of a software indoscopy which concern a hospital institution: - minimum amount of €50,000.00 VAT per certificate for the last 3 years;
