- Na zawartość oferty składa się wypełniony i podpisany Formularz Ofertowy sporządzony zgodnie ze wzorem stanowiącym załącznik nr 3 do SWZ wraz z Formularzem „Parametry oraz warunki oferowane” sporządzonym zgodnie ze wzorem stanowiącym załącznik nr A do Formularza ofertowego. Dokumenty określone powyżej stanowią ofertę, w związku z tym nie będą podlegały procedurze uzupełnienia, określonej w art. 128 ust. 1 ustawy.
- Do oferty należy załączyć oświadczenia, dokumenty i pełnomocnictwa wskazane w rozdziale VII pkt 1 SWZ, tj.: 1.1. Oświadczenie o braku podstaw do wykluczenia, o którym mowa w art. 125 ust. 1 ustawy Pzp, w zakresie wskazanym w załączniku nr 4 do SWZ. Informacje zawarte w oświadczeniu stanowić będą potwierdzenie, że Wykonawca nie podlega wykluczeniu z postępowania. 1.2.Odpis lub informację z Krajowego Rejestru Sądowego, Centralnej Ewidencji i Informacji o Działalności Gospodarczej lub innego właściwego rejestru, w celu potwierdzenia, że osoba działająca w imieniu Wykonawcy jest umocowana do jego reprezentowania.Wykonawca nie jest zobowiązany do złożenia dokumentów, o których mowa, jeżeli Zamawiający może je uzyskać za pomocą bezpłatnych i ogólnodostępnych baz danych, o ile Wykonawca wskazał dane umożliwiające dostęp do tych dokumentów. 1.3.Pełnomocnictwo lub inny dokument potwierdzający umocowanie do reprezentowania Wykonawcy, jeżeli w imieniu Wykonawcy działa osoba, której umocowanie do jego reprezentowania nie wynika z dokumentów, o których mowa w pkt 1.1. 1.4.Pełnomocnictwo (lub inny dokument, z którego wynika pełnomocnictwo) dla pełnomocnika do reprezentowania Wykonawców występujących wspólnie w postępowaniu o udzielenie zamówienia lub reprezentowania w postępowaniu i do zawarcia umowy w sprawie zamówienia publicznego.
